Residents: $10.00 per person
Non-Residents: $15.00 per person
Badgercare/Medicaid: Fee exempt

Immunization Schedule:
AGE VACCINES
Birth Hep B
2 months DTaP, Hib, Hep B, Polio, Prevnar, RotaTeq
4 months DTaP, Hib, Polio, Prevnar, RotaTeq
6 months DTaP, Hep B, Polio, Prevnar, RotaTeq
12 - 15 months DTaP, Hib, Prevnar, MMR, Varicella, Hep A
18 - 24 months Hep A
4 - 6 years DTaP, MMR, Polio, Varicella
11– 14 years Tdap, HPV, Meningococcal

Adults need:
Tetanus/Diphtheria (Td) booster every 10 years.

Influenza Vaccine:
Flu shots are recommended for: (www.cdc.gov/flu)
  • Children aged 6 months up to their 19th birthday
  • Pregnant women
  • People 50 years of age and older
  • People of any age with certain chronic medical conditions
  • People who live in nursing homes and other long-term care facilities
  • People who live with or care for those at high risk for complications from flu, including health care workers, household contacts of the above groups or caregivers of children less than 6 months of age
Tuberculosis (TB) Skin Tests:
Tuberculosis (TB) Skin Tests are done by appointment only. Please call 262-835-6429 to schedule an appointment. Cost is $10.00.
